• Tour permit (Local) - (For trips and camps under 500 miles, 3 pages, best when printed on legal [8.5 x 14] paper).
  Tour permit can only be processed with the original signatures (the actual hand written). Any tour permit turned in with computer generated signatures will be denied.
  
• Personal Health & Medical Record (Class 3) - (Not for summer camp, unless over 40 years of age.  For the summer camp medical form please see below).

  (Please note: This is a new form for the 2008 Summer Camp season.  Any older forms will no longer be accepted).

• Firearms Permission Form
  (for B.B. guns, rifles and shotguns outside of the summer camp season)
• Parental Informed Consent Agreement for Climbing/Rappelling Activities
  (Needed for weekend use and non-scout groups)
